If you are planning on using insurance to pay for your sessions, please contact your insurance company prior to your initial session for information on how your plan covers outpatient nutrition therapy.

Here are some questions to ask when contacting your insurance company about your benefits:

Does my insurance plan cover outpatient Medical Nutrition Therapy (MNT procedure codes 97802 & 97803)?

Is there a limit on the number of sessions (or units) covered?

Do I have an out-of-pocket max or a deductible that I have to meet first?

For all other insurance plans: Most insurance plans offer out-of-network (OON) benefits and will reimburse a portion of nutrition therapy services. I can provide you with a monthly statement (“superbill”) that you may submit to your insurance company for reimbursement.

I do not accept Medicaid or Medicare.
FEES
Private pay rates (not using insurance, must be paid at time of service):

Initial Nutrition Intake and Assessment (75-90 minutes): $225

Follow Up Nutrition Therapy Session (30/60 minutes): $90/$180

If you are unable to attend a scheduled appointment, please let me know at least 24 hours prior to the start of scheduled appointment time. No-shows and appointments cancelled with less than 24 hours notice will be charged the full session fee. If you arrive late to a scheduled appointment, the session will still end at the originally scheduled time.

A credit/debit card on file is required to hold your appointment. Insurance co-pays/coinsurance, late cancel/no-show fees, and private pay fees will be charged to your card unless you make other payment arrangements.
